This form is also used for Bulletin and Website Classified Ads.

If you are submitting the form via email, please do not include credit card information if you do not have encrypted email.  In place of credit card information, type: "will call with credit card".  Then save (or print and scan and save) the completed form and attach it to an email.  In the body of the email, include your name and telephone number as well as the type of ad you're placing.  Send the email to office@nassaudental.org.  After sending the email, please call our office at 516-227-1112 to provide your credit card information.

If you are submitting the form via regular mail, print the completed form and send it back with payment in the form of a check or include credit card information.  Mail it to:
